2010 SPPPA Fellowship Degree
Points Claim Form


The administrator will log your points, but it is up to you to fill out form, and track your point progress.
Inform the administrator when you become eligible for the Assoc. or Full fellowship, before the 2010 Christmas Party and your point total will be verified.

Associate Fellowship is obtained by accumulating 50 points,
Full Fellowship is obtained by accumulating 100 points.
You'll receive one star on your degree ribbon for every 25 points after achieving Full Fellowship.

You may only claim points accumulated in 2010, unless they are market by a *, signifying a grandfathered point.
